Go, Deep Portage!
Today, the Deep Portage paperwork was sent home with your child. Students should return the "Health/Permission" form and the "Rules & Guidelines" bottom portion. Included in the pack was the "What to Bring" sheet. All of the forms are available online as well by clicking on the Deep Portage link on this blog. Paperwork needs to be turned in by Friday, September 17th.
Remember - Medication forms need to be filled out and signed by the doctor for all types of medication, even over the counter medication. The clinic may fax the form to the school with the fax number on the top right hand corner of the sheet. The medication form is also available online.
